What is medical coding?
Medical coding is the translation of healthcare diagnoses, procedures, services, and supplies into universal numerical and/or alphanumerical codes. The diagnoses and procedure codes are taken from medical record documentation, such as transcription of physician’s notes, laboratory and diagnostic tests, etc.
What is the purpose of medical coding?
These medical codes are used on the claim forms submitted to the insurance companies, such as Medicare and Medicaid for reimbursement and on the statistical reports, which are used by the Federal Government of USA. These are also used internally at health care facilities for research and marketing purposes.
How the medical codes to be assigned?
The medical coder has to study the medical record and understand the patient’s condition in detail and has to assign the appropriate numerical and/or alphanumerical codes by following the guidelines and client requirements keeping in mind. Diagnosis codes are found in the International Classification of Diseases, Tenth Revision, Clinical Modification (ICD-10-CM Volumes 1 and 2). Inpatient procedure codes are found in ICD-10-PCS. Outpatient services and procedure codes are found in the Current Procedural Terminology (CPT). HCPCS (pronounced “hik picks”) codes are used for transportation services, Medical and Surgical Supply codes, Durable Medical Equipments.
